|ICD-10-CM Coding & Documentation Workshop

|Years after implantation of ICD-10, many still struggle with proper coding and sequencing. While advancement has been made to improve clinical
documentation, there is still much progress needed.
This one-day workshop will detail known problematic areas involving ICD-10-CM coding and sequencing and look at how documentation can be
improved. Join us for this interactive workshop that contains lecture, practice, discussion, review and case studies.
As always MCHC workshops are AAPC Approved for CEUs (Continuing Education Units) and by attending each participant can earn up to 7.5
CEUs. Seating for this workshop is limited at certain locations, and we anticipate it to sell out quickly.
8:00am ICD-10-CM General Coding Guidelines
Review the conventions for the ICD-10-CM. Special attention will be paid to the terminology,
punctuation and the abbreviations. Understand the differences between Excludes 1 and
Excludes 2 notes. This session will also detail signs and symptoms, acute and chronic
conditions and sequela.
9:00am Infectious & Parasitic Diseases
Chapter 1 of the chapter specific coding guidelines provides several sequencing rules such
as a patient with HIV admitted for a HIV-related condition. We will look at difference
scenarios involving B20 and how these should be sequenced. We will also look at sepsis
coding and sequencing as well as MRSA.
This session will discuss proper code assignment and sequencing of neoplasms. In addition
we will review the following:
Anemia associated with malignancy
Dehydration due to malignancy
Administration of Chemotherapy, Immunotherapy and Radiation
Secondary malignancy with unknown primary
12:45pm Diabetes Coding and Documentation
Diabetes continues to be an issue with documentation, coding and proper sequencing. This
session will clear up misconceptions and promote improved documentation. We will review
Type 1 vs. Type 2 when documentation is not provided
Insulin use and supporting code
Underdose of insulin due to pump failure
Secondary diabetes and necessary documentation
1:45pm Pain Documentation and Coding
This session will provide a detailed review of the general coding guidance for pain control
and pain management. Participants will learn the difference between acute, chronic, post
procedural and neoplasm related pain. Code G89 will be discussed and proper sequencing
of the code and supporting documentation.
3:00pm Hypertension Coding and Documentation
During this portion of the training we will outline strong documentation involving hypertension
and associated manifestations. A detailed review of hypertension with heart involvement
and/or renal involvement will assist in the understanding needed documentation
improvements and code sequencing.
4:00pm Obstetrics Coding
We know that chapter 15 codes have sequencing priority over codes from other chapters,
but we will expand and look at sequencing of some specific scenarios that have other
sequencing rules, such as HIV with a related condition during pregnancy. This session will
review the selection of the principal diagnosis and detail the use of code Z3A.
*Case Studies will be used in each of the segmented sessions to illustrate proper coding and
sequencing and to allow for discussion on improved documentation.
